Aplicación de sistemas tópicos de presión negativa en heridas y defectos de cobertura en extremidades traumatizadas

Abstract

Introduction: A delay in the time of management of wounds and post-traumatic soft tissue injuries with coverage defects is being widely described as a result of several different reasons. Negative pressure wound therapy systems are easy to apply and a well described therapeutic alternative traumatic for orthopedic injuries with good results, few adverse events and complications as reported worldwide.
Methods: We performed a prospective observational study evaluating the expected results of a 9 patients series with limb injuries and coverage defects of bone tissue, muscle, tendon and osteosynthesis materials. We used those systems to apply topical negative pressure in order to enhance the healing process.
Results: 9 patients were treated; the average age was 50.7 years, 66% male. It was evident active infection of the wound or defect coverage in 66% of cases. In 55% of the patients there was osteosynthesis implant exposure. The average time using the VAC in our series was 12.8 days. It was achieved a decrease in initial length average of 45% (2.8 cm), width average of 44% (2.3 cm) and depth average of 59% (3.5 cm). Patients with active infection showed a higher percentage of decrease of the width and depth average of wounds.
Discussion: VAC systems are useful for stimulating the formation of viable granulation tissue in the short term, as well as reduce the amount of exudate, especially in cases with active infection.
