Clinical and sociodemographic characterization of patients with femoral shaft fractures in the Colombian Caribbean and exploration of the influence of these characteristics on the presence of concomitant ipsilateral femoral neck fracture

Abstract

Introduction: Femoral shaft fractures (FDF) are severe injuries that can lead to important morbidity and mortality. If they are associated with femoral neck fractures (FCF), they often warrant modifying the approach and represent a challenge for the surgeon.

Objective: To describe the epidemiological characteristics of patients with diaphyseal femur fractures in the Colombian Caribbean region and explore the effect of these characteristics on the presence of concomitant ipsilateral FCF.

Methodology: Retrospective observational study including subjects diagnosed with diaphyseal femur fractures confirmed by computed tomography. Sociodemographic and clinical characteristics of patients with femoral shaft fractures were analyzed. For bivariate analysis, the different independent variables were compared according to the presence of ipsilateral femoral neck fractures.

Results: The male sex was predominant (60.6%). The median age was 31 years (IQR: 57). Intracapsular femoral neck fractures were recorded in 13 patients (18.3%), 52.1% of patients with femoral neck fractures had 32A femoral shaft fractures. Only the gender variable showed statistically significant differences between subjects with and without ipsilateral femoral neck fractures (p=0.011).

Conclusion: FDF were predominantly observed in young men without comorbidities and were mainly caused by traffic accidents. This study recorded a high proportion of FDF with concomitant ipsilateral FCF. The use of relevant diagnostic tools, such as computed tomography of the hip, postoperative radiography, and rigorous clinical evaluation, plays an important role in the timely identification of this association.
